Applying for Catamount Health, VHAP, and Dr. Dynasaur

This application can be filled out on your computer, but it must be printed and mailed to the state at the address on the application. You cannot apply online at this point.

Vermonters should use this form if they are:

• Single and make less than $33,000/yr
• Married with no kids and have a household income of under $43,000/yr
• Married with one child and have a household income of under $54,000/yr
• Married with two children and have a household income of under $65,000/yr.

NOTE: There is no residency requirement necessary to apply for these programs. A household is generally defined as married individuals and their children under 21 living in the same house.

Applying Directly for Catamount Health

Vermonters who are over the income thresholds noted above can apply for Catamount Health without premium assistance which costs $442/mo.

Although this application can also be used to apply for Catamount Health with premium assistance, Vermont Health Access Plan (VHAP) and Dr. Dynasaur, it requires more detailed information than necessary for those programs.
