Applying for Catamount Health, VHAP, and Dr. Dynasaur

You can apply online for Catamount Health with premium assistance, Vermont Health Access Plan (VHAP) and Dr. Dynasaur at mybenefits.vt.gov.  If you apply online, make sure you keep your confirmation number.
 
If you prefer to mail your application, you can Click here for the application for: Catamount Health with premium assistance, Vermont Health Access Plan (VHAP) and Dr. Dynasaur. This application cannot be filled out on your computer, but it must be printed, filled out, signed and mailed to the state at the address on the application. Make a copy of your application to keep for your records before mailing it in.

Vermonters should use this form if they are:

• Single and make less than $33,200/yr
• Married with no children or single parent with one child and have a household income of under $44,600/yr
• Married with one child or single parent with two children and have a household income of under $56,000/yr
• Married with two children or single parent with three children and have a household income of under $67,400/yr.

NOTE: There is no residency requirement necessary to apply for these programs. A household is generally defined as married individuals and their children under 21 living in the same house.

Applying Directly for Catamount Health

Vermonters who are over the income thresholds noted above can apply for Catamount Health without premium assistance which costs $454 for Catamount Blue (Blue Cross Blue Shield) or $513 for MVP Catamount Choice.

Applying for Medicaid

Click here for the application for Medicaid. Medicaid has income and resource limits. Although this application can also be used to apply for Catamount Health with premium assistance, Vermont Health Access Plan (VHAP) and Dr. Dynasaur, it requires more detailed information than necessary for those programs. This application will need to be printed and mailed to the state at the address on the application.
