Preparing for Your First Visit

To ensure your initial appointment is thorough and productive, we recommend bringing the following items. If you do not have all documents available, please come anyway. Our team will work with whatever information you can provide.

Identification & Insurance

  • Photo identification (driver's license, state ID, or passport)
  • Insurance card (front and back) if applicable
  • Medicare, Medicaid, or DVHA Green Mountain Care documentation
  • Proof of income for sliding-scale fee determination (recent pay stub, tax return, or benefits letter)

Medical Records

  • List of current prescription medications, including dosages and frequency
  • Known allergies to medications, foods, or environmental agents
  • Summary of recent hospitalizations or surgeries within the past five years
  • Immunization records if available, especially for patients under 26 or over 60

Personal Health Notes

  • Written list of questions or concerns you want to discuss with your provider
  • Family medical history (parents and siblings), particularly for heart disease, diabetes, and cancer
  • Recent lab results or specialist reports if transferring from another provider
  • Wellness Risk Assessment results if you completed our online screening

What to Expect at Your First Visit

Your initial appointment at Catamount Health typically lasts between 60 and 90 minutes, depending on the complexity of your health history and the services you are seeking. We intentionally schedule longer first visits so that our clinical team has ample time to listen to your concerns, conduct a thorough assessment, and begin building a care plan that reflects your individual goals and circumstances.

Upon arrival at our Montpelier center at 238 State Street, Suite 310, you will check in with our front desk staff, who will verify your identification and insurance information, process any sliding-scale fee documentation, and provide you with our HIPAA Notice of Privacy Practices and Patient Rights acknowledgment form. We ask that you arrive approximately 15 minutes before your scheduled appointment time to complete these administrative steps so that your clinical time is fully devoted to your health.

Your visit will begin with a comprehensive health history intake conducted by a medical assistant or registered nurse, including vital signs measurement (blood pressure, heart rate, temperature, weight, and height), a review of your current medications and allergies, and a brief overview of your chief concerns for the visit. You will then meet with your assigned primary care provider, who may be a board-certified family medicine physician, a certified nurse practitioner, or a physician assistant, depending on your scheduled service.

During the clinical portion of your visit, your provider will conduct a focused or comprehensive physical examination as appropriate, review any available prior medical records or lab results, and discuss evidence-based recommendations for preventive screenings, vaccinations, behavioral health support, or chronic disease management. If laboratory work is indicated, our on-site phlebotomy team can typically perform blood draws during the same visit, with results available within three to five business days.

Before you leave, your care coordinator will review your individualized care plan, schedule any necessary follow-up appointments, provide referral documentation if specialist consultation is warranted, and ensure that all of your questions have been addressed. We also encourage all new patients to explore our community wellness programs, many of which are offered at no additional cost as part of our mission to promote population health throughout central Vermont.

New Patient Questions

Do I need a referral to become a patient?

No referral is required for most Catamount Health services. You may self-refer for wellness exams, behavioral health intake, nutrition counseling, and community programs. Certain specialist services or chronic disease management programs may benefit from coordination with your current provider, but this is never a barrier to accessing care.

What if I do not have insurance?

Catamount Health serves all patients regardless of insurance status. Our sliding-scale fee schedule adjusts costs based on household income and family size, with discounts ranging from 15% to 75% off standard rates. Patients at or below 100% of the Federal Poverty Level qualify for our Hardship tier with a 75% reduction. Our billing team can also assist you in applying for Vermont Medicaid or Green Mountain Care if you may be eligible.

Can I request a specific provider?

Yes. When completing the intake form, you may note a provider preference in the reason for visit field. We will do our best to accommodate your request based on availability. If your preferred provider is unavailable for your initial visit, our coordination team will schedule a follow-up with them and assign a qualified colleague for your first appointment.
